Thank you to everyone who made this journey in our facility through the pandemic – our teams (staff, contractors, vendors and providers) and the residents, family and friends. The Public Health Emergency for COVID-19 has ended as of May 11, 2023.

Our Guest book is now electronic on an iPad at the entrance. Should you choose to bypass the iPad, your actions affirm you have chosen you are fully educated and accept the risk of entering. We encourage you to use it as it allows us a possibility to contract trace versus testing the entire facility should a case be identified. We would love to give the residents and staff a break.

Our Community

Set in rural southwest Iowa just 40 miles southeast of Council Bluffs, IA, and Omaha, NE, Red Oak provides high-quality transitional and long-term skilled nursing care. Our location allows for convenient access to Montgomery County Memorial Hospital, part of the Omaha-based Methodist Health System. We are also affiliated with Shenandoah’s Critical Care Hospital, part of the Des Moines-based Mercy Health System.

Our Approach

We understand that moving from a familiar environment to a new community is a big change. We’re here to help you every step of the way.

Before you move in, we’ll develop a personalized health and wellness plan that includes a pre-admission appointment with your physician. The plan also includes your individual preferences, lifestyle interests and goals. Once you arrive, our transition team helps you thrive in your new home with services that include:

Medication management
Assistance with bathing and dressing
Wound care
Coordinated services with outside healthcare professionals
Recreational and social programming
Housekeeping, laundry service, and apartment maintenance
Transportation coordination
Three meals a day and snacks
Therapy services, including: physical, occupational, and speech

Additionally, there are options for utilizing in-house services such as:
